2021 Breakthrough Fort Worth Application Information

Due to COVID-19 restrictions, the Breakthrough Fort Worth application process will be on a rolling admission basis starting February 3, 2021. Applications will be reviewed as they are received and candidates evaluated accordingly until all spots are filled.

Application Dates

  • February 3, 2021 - Online Applications go LIVE at
  • April 16, 2021 - Application will close
  • May 1, 2021 - All decision and final notifications will be sent by May 1

How to apply

You will need to have or create a free Google account in order to upload using this form. You can create a Google account with your current e-mail address. Otherwise, you can e-mail your student’s most recent report card to Once we receive your report card, we will begin to consider your application.

Part 1

Complete the Online Application Part 1 - Must be completed by the Parent/Guardian
    1. General Student Information
    2. Parent/Guardian Information
    3. Family/Breakthrough Partnership Agreements
    4. Most Recent Report Card
    5. Email Address for your Recommenders
Upon completion of Part 1, you will be sent an email with instructions on how to submit the student supplement.

Part 2

Student Supplement - Must be completed by the student
    1. Essay
    2. Short Answer
    3. “Express Yourself” Video 

Part 3

Student Interview - If selected to advance, a Breakthrough staff member will contact you to schedule a student interview.

If you would like to know more about the program, the application process, and selections, please attend one of our information sessions. All sessions will be held via Zoom.
