In this time of uncertainty, Fort Worth Country Day understands that obtaining an appointment for your student’s annual physical may be difficult. While the School will still need both the physical and immunization form for 2020-21 enrollment, parents with new students may now complete a medical waiver (with a set appointment date) by July 1 in lieu of the actual physical. Your student’s most current immunization record must be submitted at this time as well; you will provide the updated form following your physician visit. Returning students are on a rolling expiration date and will submit their health paperwork following their next physical. Click on the link for the 2020-21 medical waiver.

Falcon Sports Program Advertising - Reserve Space Now
Each year the FWCD Falcon Club publishes three, four-color seasonal (fall, winter, spring) sports programs that include advertisements and sports information on all Upper School athletic teams.
The programs are a great opportunity to highlight your business. Ad sales help the Falcon Club support our student-athletes and coaches, as well as the FWCD athletic program. Ads run in all three seasonal programs.
